In order to be seen by a healthcare provider or to be treated in our facility, please call Student Health Services at 239-590-7966 to schedule an appointment. If you need to make an immunization appointment, call 239-590-1254.
You may also come in and schedule an appointment that meets your needs. Student Health Services is adjacent to Parking Garage 2, behind the Arts Complex. You will need to complete some initial paperwork prior to being seen by a provider. If you are coming in the same day as your visit, please arrive 10 to 15 minutes early to fill out paperwork. You may also pick up paperwork in advance and bring it back at the time of your appointment.
- You must bring your Eagle ID with you for all appointments.
- Missed appointments not canceled at least 2 hours ahead of the appointment time are subject to the $25 no-show fee.
- Bring a form of payment for any nominal charges you may incur at the time of your appointment. Student Health Services takes all major forms of payment including Visa, Mastercard, American Express and Discover. Your Eagle ID can also be used for payment if there are existing funds associated to the card; charges also can be posted to your Gulfline account.
